Ascend Support
At Ascend Support, we provide lower-dependency community services and outreach services that help to facilitate hospital discharge.
Some clients can reach independence, move on into the community, and live independently with arm’s length/outreach support. This can be different for everybody, but the aim is to give “just enough support” to people so they can live meaningful lives as part of their local communities, with support to help maintain independence as needed.
Ascend Support also enables others who may need support in their own homes to live as independently as possible with the assistance of qualified professionals who offer excellent support.
For people with a predominant mental health need, Ascend Support plans and adjusts support as needed, working with external professionals to mitigate any developing crisis, allowing further assessment and planned support. This helps people who have cyclical mental health needs, allowing them to maintain independence as much as possible with appropriate support. The consequence of our support is a direct reduction in the continued use of statutory community services and the removal of the potential need to enter higher levels of support and more restrictive service models, avoiding further escalation of presentation.
